If you have paid for the App from transformhq.com or thetransformapp.com, you will then need to install the app on your mobile device from the app store. 

Once you get through some of the onboarding questions it will pop up a payment screen.

DO NOT PROCEED TO PURCHASE a new subscription.

You have already done this through one of the websites. You will want to select the button "Already Paid."

This will pass you through the payment screen to be able to start your transformation.
